*Any child requiring medication during the school day (EpiPen, EpiPen Jr.,inhalers, nebulizer solution, daily medications) must have a completed Medication Administration form with a physician's signature on file.  Medication MUST be provided by the family in the pharmacy labeled container. Allergy and Asthma Action plans are also available for download. Please be sure to check expiration dates on medications. Attach a photo of your child (small head shot) to emergency medication orders.

*Medications that will be used for a short course (ten days or less) do not require a Medication Administration form signed by the physician.

What to do if your child is ill or sent home from school ill (Guidelines)

Physical Examinations- Documentation of a current physical examination (within 12 months prior to first day of school entry and at intervals of every three to four years thereafter) must be on file in the health office. Any student wanting to participate in sports must have a yearly physical examination on file in the health office prior to the start of the season. Please check with the school nurse to make sure your child is in compliance.

Documentation of the listed vaccines is required by the Department of Public Health. These must be on file in the health office prior to the start of the 2017-2018 school year unless there is religious or medical exemption.  Updated information may be faxed to (508) 278-0391. Any claim of religious or medical exemption must be submitted in writing to the health office at the beginning of each school year.
